Locating a participating Landmark practitioner in your area is easy. To begin, use the dropdown menu below to choose your Plan Type, the Practitioner Type* you wish to see, and your State. Then continue to narrow your search by selecting one of the following: City, Zip Code, County, or Practitioner Name.

*Note: The Practitioner Type must correspond to your benefit plan. For example, if you have a chiropractic benefit, your Practitioner Type will be a chiropractor. If you have an Acupuncture benefit, your Practitioner Type will be an acupuncturist, etc.

Please confirm your selected practitioner's participation status when you call to schedule an appointment.
